Feedback and Complaints Form

As an NDIS participant, you have the right to make complaints about our services and supports at any time.

To make a complaint or provide feedback, you can complete our Feedback and Complaints Form. It will be sent directly to our Human Resources and Compliance Manager. Alternatively, you can email her at hr@kapitalcare.com.au.

We will handle your feedback and/or complaint fairly, following the steps outlined in our Complaints Management System.

We are happy to provide you with a copy of our Complaints Management Policy at any time. This can be sent to you via email or provided as a hard copy. Please email us or call us to request a copy.

If you would prefer to provide your feedback or complaint in another way, you may do so by:

  • Talking to us face-to-face

  • Calling us on the phone

  • Using your preferred Augmentative or Alternative Communication (AAC) method

  • Sending an email

  • Sending a text message

To protect your privacy, we recommend avoiding social media or discussing complaints directly with support staff, as this may prevent your concerns from reaching the appropriate team at Kapital Care.
